Mississippi Health First’s mission is to address the incidence of health disparities and to increase health literacy and diabetes self-management to improve health outcomes. The initiative will be characterized by a partnership between Federal and non-federal entities in order to maximize resources and affect the largest possible number of individuals with diabetes. The collaborative partnership consists of members from the following Federal and non-federal entities:

Mississippi Health First’s main goals are to recruit and retain participants and providers; establish relationships at the local level; measure clinical outcomes; and improve the value of individual programs with a larger collaborative program. What is unprecedented is its approach which includes the collaboration of Federal and non-federal partners as well as the hope for many additional partners, especially at the State and local level, including faith-based organizations.
