Your assistance is needed in referring all Medicare beneficiaries with diabetes for diabetes self-management education (DSME) training. The Every Diabetic Counts program is a patient-centered approach to DSME that addresses the cultural, social, and behavioral concerns affecting people with diabetes.
According to the Centers for Disease Control and Prevention (CDC), underserved (African-American, Hispanic/Latino, American Indian/Alaska Native and Asian/Pacific Islander) populations experience a disproportionate incidence of diabetes, including excess health utilization and costs. Every Diabetic Counts will serve these populations in order to help patients become more active in their diabetes care, improve their health, and ultimately reduce disparities one patient at a time. |
