MISSISSIPPI HEALTH FIRST
Mississippi Health First (MHF) is a project that provides adults living with diabetes in the state of Mississippi the opportunity to participate in free diabetes self-management education (DSME) classes. These classes are offered statewide and conducted at locations such as senior public housing centers, state area agency on aging community centers and federally qualified health centers. Mississippi Health First’s mission is to address the incidence of health disparities and to increase health literacy and diabetes self-management to improve health outcomes. This initiative is characterized by a partnership between Federal and non-federal entities for maximized resources and reach. The collaborative partnership consists of members from the following Federal and non-federal entities:
- Administration on Aging (AoA)
- American Association of Diabetes Educators (AADE)
- American Diabetes Association (ADA)
- Centers for Disease Control and Prevention (CDC)
- Centers for Medicare & Medicaid Services (CMS)
- National Institutes of Health (NIH)
- Health Resources and Services Administration (HRSA)
- Department of Housing and Urban Development (HUD)
- National Academy for State Health Policy (NASHP)
- HHS/Office of Minority Health (OMH)
The main goals of the Mississippi Health First initiative are:
- To recruit and retain participants and providers;
- Establish relations at the local level;
- Measure clinical outcomes; and
- Improve the value of individual programs with a larger collaborative program.
