EDC HEALTHCARE PROFESSIONALS
Your assistance is needed in referring all underserved Medicare beneficiaries with diabetes for free diabetes self-management education (DSME) training. The Every Diabetic Counts (EDC) program is a patient-centered approach to DSME.
DSME is the ongoing process of facilitating the knowledge, skills, and abilities necessary for diabetes self-care. This process incorporates the needs, goals, and life experiences of the person with diabetes and is guided by evidence-based standards. The DSME training helps Medicare beneficiaries with diabetes learn how to manage the disease and live longer, healthier lives.
According to the Centers for Disease Control and Prevention (CDC), underserved populations (African-Americans, Hispanics/Latinos, American Indians/Alaska Natives and Asian/Pacific Islanders) experience a disproportionate rate of diabetes, as well as additional health utilization and costs to manage their disease. The EDC program serves these populations in order to help people with diabetes become more active in their diabetes care, improve their health and ultimately reduce health disparities one patient at a time. The goal of the EDC program is to lower Hemoglobin A1C and cholesterol levels, blood pressure as well as obtain an annual eye examination.
