CERTIFIED DIABETES EDUCATORS

The certified diabetes educator (CDE) will train Medicare beneficiaries using a train-the-trainer model to achieve the goals of the Every Diabetic Counts (EDC) program. CDEs provide the technical expertise and teach the clinical portion of the diabetes self-management (DSME) curriculum. All job duties are performed in a manner that demonstrates the values of care, respect, and professional excellence toward patients, families, volunteers, physicians, and co-workers.

Essential duties may include:

  • Coordinating and providing DSME for Medicare beneficiaries ;
  • Coordinating activities of the EDC program with project managers;
  • Promoting the DSME program;
  • Performing other duties as directed in a cooperative and efficient manner;
  • Pursuing professionalism, clinical motivation, and participation in continuing education to ensure effectiveness; and
  • Adhering to confidentiality policies and procedures without exception.
